This case report highlights the diagnosis, surgical management, and outcome of an abdominal teratoma in a young child treated at our pediatric surgical center in Ghaziabad. Teratomas are rare, germ cell tumors that may contain tissue from all three embryonic layers and can occur in various anatomical locations. Abdominal teratomas, although uncommon, can pose significant risks if not diagnosed and treated early.

Patient Profile:

  • Age: 8-month-old female

  • Presenting Complaint: Abdominal distension and a palpable mass in the lower abdomen for 2 weeks

  • Other symptoms: Mild vomiting, poor feeding, no fever

Clinical Evaluation:

  • Physical Examination: Non-tender, firm mass in the left lower quadrant of the abdomen

  • Investigations:

    • Ultrasound Abdomen: Large heterogeneous mass with cystic and solid components

    • CT Scan: Well-defined retroperitoneal mass measuring 8×6 cm, displacing adjacent bowel loops

    • Tumor Markers: AFP mildly elevated, β-hCG normal

Provisional Diagnosis:

  • Abdominal (retroperitoneal) teratoma – likely mature, non-malignant

Surgical Management:

  • Procedure: Elective exploratory laparotomy under general anesthesia

  • Findings: Large encapsulated mass arising from the retroperitoneum, adherent to adjacent mesentery but not involving major vessels

  • Surgery: Complete excision of the tumor without spillage

Histopathology Report:

  • Diagnosis: Mature cystic teratoma

  • No evidence of malignant transformation

Postoperative Course:

  • Uneventful recovery

  • Discharged on postoperative day 5

  • Follow-up ultrasound at 3 months – no recurrence

  • Tumor markers returned to normal range

Conclusion:

This case emphasizes the importance of early detection and surgical intervention in managing pediatric abdominal teratomas. With skilled pediatric surgical care and modern imaging, outcomes are excellent, especially for mature teratomas.
