Congenital Pulmonary Airway Malformation (CPAM), previously called Congenital Cystic Adenomatoid Malformation (CCAM), is a non-cancerous developmental abnormality of the lung. It involves the growth of abnormal cyst-like lung tissue that does not function properly in gas exchange.
CPAM can vary in size and location and typically affects one lobe of the lung. It may be detected before birth during routine prenatal ultrasound or become noticeable after birth due to breathing difficulties.
Infants and children with malrotation may exhibit the following signs:
In utero (before birth):
Detected as a lung mass or cyst on prenatal ultrasound
May lead to hydrops fetalis (severe fetal swelling) in rare cases
After birth:
Rapid or labored breathing
Recurrent lung infections
Poor feeding
Cough or respiratory distress
In some cases, CPAM may remain asymptomatic and found incidentally on imaging
Diagnostic tools include:
Prenatal ultrasound and fetal MRI
Chest X-ray and CT scan after birth
Echocardiogram to assess heart involvement
Blood oxygen levels and respiratory assessment
Treatment:
Surgical removal (lobectomy) of the affected lung lobe is recommended to:
Prevent future infections
Allow normal lung development
Eliminate the rare risk of malignancy later in life
Minimally invasive (thoracoscopic) surgery is preferred for quicker recovery and less discomfort
In asymptomatic cases, surgery is still advised in infancy to avoid complications as the child grows.
