Acalculous Cholecystitis is an inflammation of the gallbladder without gallstones, often seen in critically ill or post-surgical patients, or those with trauma, infections, or long-term fasting. It can be life-threatening if not diagnosed early and managed appropriately.
Unlike typical cholecystitis caused by gallstones, this condition arises due to reduced blood flow (ischemia), bile stasis, or infection, leading to gallbladder wall inflammation and sometimes necrosis.
Critically ill patients (ICU admissions)
Severe trauma or burns
Sepsis or major infections
Long-term intravenous nutrition (TPN)
Post-surgical patients
Diabetes or vascular diseases
Sudden, severe upper right abdominal pain
Fever and chills
Nausea or vomiting
Bloating or indigestion

Abdominal tenderness, especially in the right upper quadrant
In critically ill or unconscious patients, symptoms may be subtle, and diagnosis relies on imaging.
Diagnostic tools include:
Ultrasound (thickened gallbladder wall, fluid around gallbladder)
CT scan for detailed assessment
HIDA scan (to evaluate gallbladder function)
Blood tests for infection and liver function
Treatment options:
IV antibiotics to control infection
Supportive care (fluids, pain relief, nutritional management)
Cholecystostomy (drain insertion) for high-risk surgical patients
Laparoscopic cholecystectomy (gallbladder removal) for stable patients
Prompt treatment is essential to prevent gallbladder rupture, gangrene, or sepsis.
