What is a Hydrocele?

A hydrocele is a fluid-filled sac around the testicle within the scrotum. In children, it usually results from a congenital connection between the abdomen and the scrotum (called the processus vaginalis), which allows fluid to pass down. This condition is especially common in newborns and infants.

Types of Hydroceles

  • Communicating Hydrocele: Has a connection with the abdominal cavity; swelling may vary throughout the day.

  • Non-communicating Hydrocele: No abdominal connection; swelling remains constant.

Symptoms of Hydrocele

  • Painless swelling in one or both sides of the scrotum

  • Size may increase during the day or with crying (in communicating type)

  • Soft, fluid-like feel on touch

  • No redness or tenderness unless infected

  • Usually not associated with fever or discomfort

Diagnosis and Evaluation

  • Clinical examination by a pediatric surgeon

  • Transillumination test – light passes through the fluid-filled sac

  • Ultrasound of the scrotum to rule out hernia or testicular issues

Treatment Options

Observation:

  • In infants, hydroceles often resolve by 12 to 18 months of age.

  • Regular monitoring by a pediatric surgeon.

Surgical Repair:

  • Recommended if:

    • The hydrocele persists beyond 1–2 years of age

    • It is large or causes discomfort

    • Associated with a hernia

  • A day-care surgery under general anesthesia

  • Small incision made in the groin; fluid drained and sac closed

  • Fast recovery – most children resume normal activity within a few days
