What is PSARP?

Posterior Sagittal Anorectoplasty (PSARP) is a precise and reconstructive surgical procedure used to treat anorectal malformations—a group of birth defects where the anus and rectum do not develop properly. In many cases, the rectum may be improperly connected to the urinary or genital tract, or the anal opening may be absent or misplaced.

Conditions Treated with PSARP

  • Imperforate anus (no anal opening)

  • Rectourethral fistula (rectum connected to the urethra)

  • Rectovestibular fistula (rectum opening into the vagina in girls)

  • High and intermediate anorectal malformations

Symptoms of Anorectal Malformations

How PSARP is Performed

  • The child is placed under general anesthesia.

  • A midline incision is made from the sacrum to the perineum.

  • The misplaced rectum is carefully dissected and mobilized.

  • A new anal opening is created in the correct anatomical position.

  • The rectum is precisely aligned and sutured into the muscle complex to allow voluntary bowel control.

Depending on the condition, the procedure may be staged with a temporary colostomy before final repair.
