What causes hydrocele in children?

The etiology of hydrocele in children is same as inguinal hernia i.e there is a persistent sac that leads to a communication between abdomen and scrotum. However, instead of intestines and omentum peritoneal clear fluid seeps through this communication in hydrocele leading to its collection in scrotal sac.

Does hydrocele require surgery , if yes when shall it be done?

Hydrocele unlike inguinal hernia is not an operative emergency and if detected early, surgery can wait till 1-2 years age as there are chances of spontaneous resolution.

Surgery for hydrocele is same as inguinal hernia i.e herniotomy in which the sac needs to be separated from structures surrounding it like vas and vessels, transacted and proximal sac tied off. In addition, hydrocele fluid is evacuated from distal sac.

Are there any complications expected in post operative period?

Post operative scrotal swelling is relatively common which shall not be confused with recurrence of hydrocele as this generally disappears between 3-6 weeks. There are no other major complications associated with hydrocele surgery.
